Abstract

Abstract Background and Objective: Parasitic infection is one of the major health problems in the world. This study aimed at comparing the accuracy of two methods of direct examination and Formalin-Ether to detect the presence of parasitic infection among health-card applicants in Shahroud city, 2011. Material and Methods: This cross-sectional study was conducted on 801 patients seeking health-card. From each patient, three consecutive stool samples were taken and investigated, using direct examination and formalin-ether method. Results: The use of formalin-ether method in recognizing the parasitic infection specially giardia lamblia and entamobea coli is more than the direct method. Conclusion: The formalin-ether method is a more sensitive method than the direct method. But in circumstances that is urgency to respond or aims to see the shape of trophozoite, the use of direct method is recommended. Abstract

Abstract Background and Objective: Nowadays, nosocomial infection is one of the greatest problems in hospitals. Normal flora of staff’s hands and the bacterial agents on the surface of medical equipment can become progressively colonized with potential pathogens during patient care. This study was carried out to determine the bacterial agents existed on staff’s hands and in the wards of hospital to step in to control nosocomial infection. Material and Methods: In this descriptive study, during 17 months (22.mar.2010- 30.aug.2011), 403 samples, using sterile swab , were randomly obtained from the staff’s hands and medical equipment of emergency departments , ICU, male operation room and female surgical unit . The samples were cultured on Blood agar (BA) and Eosin methylene blue (EMB). Then, identification of isolated bacteria was done with diagnostic tests. Results: Of 430 samples, 530 bacteria were isolated from staff’s hands (N= 291) and medical equipment (N= 234). The most common bacterium from personnel’s hands (144 49.5%) and medical equipment (24 10%) is Staphylococcus aureus. Also, three isolates of pseudomonas aeruginosa from staff’s hands of male surgical ward and medical equipment of ICU, and two isolates of Acinetobacter.spp from ICU’s medical equipment were identified. Conclusion: With regard to the findings, it seems that applying the appropriate disinfectant agents by using standard procedures is necessary. Abstract

Abstract Background and Objective: Iron deficiency anemia is one of the most common nutritional problems in the world. This research aimed at determining the prevalence of iron deficiency anemia in one of the most endangered age group in Baft, Iran. Material and Method: this cross-sectional study was conducted on 325 high school students selected via simple random sampling (165boysand 160girls). The participants with MCV≤ 80µm or MCH ≤ 27 Pg who had simultaneously TIBC ≤ 15 % or Ferritin ≤ 1 5 ng/ml were considered as an iron deficiency anemia. Results: considering MCV and MCH, the prevalence of iron deficiency anemia in girls (14.37%) and boys (2.42%) was significantly different (P <0.05). Conclusion: The prevalence of anemia in high school students of Baft is significantly high in comparison with other studies. To increase students' intellectual and physical performance, Screening and treatment of iron deficiency are recommended. Keywords: Anemia Iron Deficiency Baft Students

Abstract

Abstract Background and Objective: Acanthamoeba species are free-living protozoa that can be isolated from all environments. They can bring about different diseases in healthy individuals and immune suppressed patients, for example, Granulomatous Amoebic Encephalitis (GAE), Acanthamoeba Keratitis (AK), Cutaneous and Nasopharyngeal infections. The aim of this study was to evaluate the Acanthamoeba prevalence in rural water sources of Markazi province. Material and Methods: In this cross sectional study, 54 water samples were collected from 36 villages of Markus province. First, the Samples were filtered by filter paper (watchman 42). Next, the filtered paper were placed in page saline solution and centrifuged. Then, the obtained sediment was cultured on non-nutrient agar (NNA) plates overlaid with heat-killed Escherichia coli. After that, the provided smear (after 4 - 7 days) stained with Geimsa. Results: The samples were positive (33 61.11%) and negative (21 38.89%) for Acanthamoeba cyst. The best result for isolation of Acanthamoeba cysts was obtained after shaking of filter paper. Conclusion: A high percentage of rural water sources have been contaminated with Acanthamoeba, which can be the major factor in causing human infections. Abstract

Abstract Background and Objective: Urinary tract infections and bacteremia are the major problems in renal transplant patients, which are mostly due to immunesuppressive regimens, surgery, and exposure to the germs in hospital. The aim of this study was to determine the prevalence of bacterial agents in the blood and urine samples of kidney transplant candidates. Material and Methods: In this one-year-long study, thirty-three renal transplant candidates were assessed for urine and blood cultures. One urine and blood samples from each patient before transplantation and three samples after transplantation were collected. The Samples, using standard microbiological methods, were investigated and infectious organisms identified. Results: In 133 urine samples, Escherichia coli (20.5%), Enterobacter spp. (5.3%), Klebsiella spp. (3 %) and Staphylococcus epidermidis (1.5%) were isolated. In the blood samples, Enterobacter spp. (9.1%), Escherichia coli (6.8%), Staphylococcus epidermidis (3.8%) and Klebsiella spp. (0.8%) were isolated. Conclusion: The results indicate that urinary tract infection was high in patients with transplanted kidney, and E. coli is the most common cause of this infection. Abstract

Abstract Background and Objective: This study was aimed to determine the extent of bacterial contamination and drug resistance patterns of isolates colonized in colonoscope and endoscope and in relevant personnel. Material and Methods: A total of 107 samples were obtained from staff of endoscopy and colonoscopy units (SEU and SCU) and gastroenterological imaging equipment. For isolation and identification of the bacteria, swab culture method and biochemical identification test were used, respectively. Antimicrobial resistance profiles, multi-drug resistance (MDR) patterns and phenetic relatedness of these isolates were also analyzed according to standard methods. Results: Most frequent pathogenic bacteria among the SEU and gastroenterological imaging related equipments were included S. aureus (20.8 % and 0 %) Enterococcus spp. (0 % and 5.4%) Pseudomonas spp. (0% and 13.5 %), and Clostridium difficile (0% and 12.5%). Analysis of resistance phenotypes showed a high frequency of MDR phenotypes among the SEU (82.1%), and also in endoscopes, colonoscopes, and other equipments (20%, 50% and 100%, respectively). Phylotyping of S. epidermidis isolates showed the role of staff in transmission of resistance strains to medical equipments and also circulation of strains with identical resistance phenotype among the studied samples. Conclusion: High frequency of pathogenic bacteria in colonoscopes, endoscopes and in the staff of endoscopy & colonoscopy units, and also contamination of these instruments with MDR pathogens emphasize the need for proper disinfection of endoscopes and colonoscopes and also instruction of staff in these units. Abstract

Abstract Background and Objective: C - reactive protein (CRP) is an acute phase protein produced in liver. It is less than 5 mg per deciliter in the serum and body fluids of normal individuals, but it is increased suddenly within a few hours following inflammatory reaction. In bacterial and viral infections, active rheumatic fever, acute myocardial infarction and rheumatoid arthritis are also increased. The aim of this study was to investigate CRP level by Qualitative and Quantitative methods. Material and Methods: The CRP of 200 patients was investigated by quantitative and qualitative methods. Qualitative CRP testing was conducted three times by different people, using two kit of bionic and Omega, and then the mean of the results was reported. For quantitative CRP testing, Immunoturbidimetry was used. Results: In qualitative CRP test by Bionic kit: 180 (90%) were negative, 6 (3%) weakly positive, 9 (4.5%) +1 and 5 (2.5 %) were + 2. In qualitative CRP test by Omega Kit: 148 (74%) were negative, 32 (16%) weakly positive, 13 (6.5%) +1, 4 (2%) +2 and 3 (1.5%) were +3. A high percentage of Qualitative results, which were weakly positive, became negative by Quantitative methods. The Qualitative results of +1 and the next became positive by Quantitative methods. Conclusion: It seems that in the early stages of inflammatory disease, quantitative methods are preferred to qualitative methods. Also, in cases that the CRP test results are weakly positive by qualitative methods, they should be controlled by quantitative methods too. Keywords: CRP CRP Test Quantitative Qualitative CRP Test

Abstract

Abstract Background and Objective: Honey is a healthy and nutritious food that has been used for a long time as a treatment for different diseases. One of the applied properties of honey is its antimicrobial effect, which differs between different types of honey due to variation of phenolic and antioxidant compositions. This study aimed to assess antimicrobial effect of honey on Bacillus cereus, considering its chemical properties. Material and Methods: Three samples of honey (A1 and A2 of Khorasan Razavi Province and A3 of South Khorasan province (were prepared and studied in terms of chemical parameters .The antibacterial effect of honey was surveyed throughTurbidimeter using spectrometer with incubator time of 2, 4, 6, and 8hrs. the level of turbidity caused by bacterium growth was measured at different times with a wavelength of 600nm. Results: According to the study, the samples containing higher concentration of polyphenol has more antimicrobial activity. The samples of A2, A3, and A1 had the highest concentration of polyphenol, respectively. Conclusion: The results indicate the prebiotic effect of honey that can be justified by the presence of fructo-oligosacharids and vitamin B. Abstract

Abstract Background and Objective: Recently, diabetes mellitus has been known as one of the main cause of upper gastrointestinal symptoms. Since a high prevalence of H. Pylori in diabetic patients has been reported, we aimed to evaluate the level of gastric juice Nitric Oxide (NO°), Oxidative Stress and Glycated Hemoglobin. Material and Methods: In case group, the participants were 60 diabetic patients infected with H. Pylori, and in control groups 60 diabetic patients without H. Pylori and 60 healthy individuals. The level of NO° in gastric juice was measured calorimetrically and the activity of superoxide dismutase (SOD) and glutathione peroxidase (GPX) in gastric biopsy was determined using standard methods. The percentage of Glycated Hemoglobin (HbA1C) was measured by ion exchange chromatography. Results: In case group compared to controls, significantly increased level of blood HbA1C, nitric oxide in gastric juice, activity of SOD and GPX in the gastric mucosa were observed (p<0.0001). Conclusion: A significant increase of glycated hemoglobin in diabetic patients with H. Pylori and high activity of antioxidant enzymes in the case group may indicate a high production of reactive oxygen species and the presence of oxidative stress in these patients. Abstract

Abstract Background and Objective: With regard to high prevalence of attention deficit hyperactivity disorder (ADHD) and its being significantly affected by nutritional factors, we aimed to determine the relationship between Ferritin serum level and ADHD. Material and methods: This ex-post- facto (causal comparative research) design study was conducted on 60 children, selected via convenience sampling. Thirty of them were ADHD children diagnosed by a psychiatrist using DSM IV checklist, as a case group, and the rest were healthy ones located in control group. Having their family informed consent, their Ferritin level was measured via ELIZA method. Results: The results show that Ferritin serum level of ADHD children are lower than that of healthy ones. Using t- test, it was indicated that the difference is significant (p= 0.002). Besides, the result of Pearson correlation coefficient showed that there is no significant relation between Ferritin and ADHD. Conclusion: In terms of the results and the importance of this issue, we recommend conducting further controlled research. Keywords: Ferritin, ADHD, Attention Deficit Hyperactivity Disorder

Abstract

Abstract Background and Objective: Malassezia that is a part of normal flora is lipophilic yeast involved in a variety of skin diseases such as seborrheic dermatitis, pityriasis versicolor, atopic dermatitis and psoriasis. Seborrheic dermatitis affects most often the sebaceous-gland-rich areas of skin such as face, scalp, and parts of the upper trunk. Dandruff is a mild variant of seborrheic dermatitis characterized by scaling. In this study, Malassezia species causing dandruff were identified. Material and Methods: In this descriptive study, the samples (n= 60) from participants with dandruff were examined under a microscope using 10% KOH solution and cultured in Leeming and Notman ager medium. DNA Extraction was performed from colonies by glass bead and the Malassezia genus, and species were detected by CfoI enzyme using PCR-RFLP method Results: Of 60, 40 (66.6%) were positive for Malassezia yeast. The positive samples in direct examination grew in culture medium. Malassezia species isolated were Malassezia globosa (25 cases), Malassezia restricta (10 cases), Malassezia furfur (3 cases) and Malassezia sympodialis (2 cases). Conclusions: In most studies, the Malassezia species were identified as the agents causing seborrheic dermatitis. In our study, Malassezia globosa was isolated as a dominant species. Abstract

Abstract In this study, the prevalence of Echinococcus granulosus was surveyed by meta-analysis and systematic review. Using main keywords of Echinococcus granulosus, dog and carnivores, we searched standard sites such as Google scholar, SID, Pubmed, Iranmedex, Irandoc, Science direct and magiran. Fifteen reliable articles were reviewed and analyzed by STATA software, Ver 11. From obtained articles, the number of 2271 dogs, 167 jackals and 174 foxes have been investigated by Iranian researchers. The infection rate of E.granulosus was reported 27.4% (95% CI, 20.9-33.9) in this study in that the highest from north and north west of Iran with prevalence of 33.3 %(CI:22.1-44.4), the lowest by 21.9 %(CI:12.5-31.3) from west and south west of Iran and the average from the central region of Iran ( 26.9% CI:12.8-41). Among cities the highest (64%) and the lowest (3.3%) infection rate were reported in Garmsar city (2010) and Sistan and Balochestan province (1997). Given that the overall prevalence of E.granulosus in Iran is high, Control and prevention measures including destruction of stray dogs and treatment of owner dogs seems to be necessary. Abstract

Abstract Background and Objective: Dermatophytes are the fungi that have the ability to attack the keratinized tissues such as the skin, hair and nails. Infections caused by these organisms are named dermatophytosis. We aimed to compare Minimum inhibitory concentration (MIC) of Crude extracts of Active Actinomycete Isolates with Terbinafine, Griseofulvin, Ketoconazole and Fluconazole Drugs against Microsporum Canis, Microsporum gypseum and Trichophyton mentagrophytes. Material and Methods: In this experimental study, in order to find MIC by actionmycete, 100 isolates were studied and then crude extracts of the active actinomycete isolates were prepared in sterile conditions. Finally, the crude extracts obtained at different concentrations were used to obtain the MIC of Microsporum Canis, Microsporum gypseum and Trichophyton mentagrophytes. Moreover, various concentrations of the drugs such as terbinafine, griseofulvin, ketoconazole and fluconazole in solvent Dimethyl sulfoxide (DMSO) were prepared and their growth inhibitory effect was evaluated and then compared with the results obtained from the crude extract of active actinomycete isolates. Results: the crude extracts obtained from active Actioiomycetes isolates and the drugs such as terbinafine, griseofulvin, ketoconazole and fluconazole, in a dose-dependent manner, could inhibit the growth of Microsporum Canis, Microsporum gypseum and Trichophyton Mentagrophytes. Conclusion: compared to MIC of Crude extract of active actinomycete isolates, Terbinafine has a significant effect on the growth inhibition in all of the fungal Dermatophytes and then griseofulvin, ketoconazole and fluconazole are in the next rank, respectively. Material and Methods:  This ex-post- facto (causal comparative research) design study was conducted on 60 children, selected via convenience sampling. Thirty of them were ADHD children diagnosed by a psychiatrist using DSM IV checklist, as a case group, and the rest were healthy ones located in control group. Having their family informed consent, their Zinc level was measured via Colorimetric method.

Results: The results show that Zinc serum level of ADHD children are lower than that of healthy ones.  Using t- test, it was indicated that the difference is not significant. Besides, the result of Pearson correlation coefficient showed that there is no significant relation between Zinc and ADHD.

Conclusion: given that the zinc level was lower in ADHD children, the difference was not significant. Therefore, we recommend conducting further research with a larger sample size.

Abstract

Abstract

        Background and Objective: Hirudin is an anticoagulant polypeptide secreted from the salivary glands of leeches. Recombinant hirudin is a strong anticoagulant agent in arterial and venous thrombosis. The aim of this study was to evaluate the effect of inserting protein A signal peptide sequence of pEZZ18 plasmid on expression and secretion of the recombinant hirudin in E.coli.

       Methods: the synthetic hirudin gene was amplified by PCR using specific primers. First, the gene was purified and cloned into PTG19-T cloning vector, and then it was subcloned into pEZZ18 expression vector by SalI / SacI enzymatic digestion and finally transformed into E.coli JM107. After the expression of recombinant hirudin protein, different cellular fractions were isolated and analyzed on SDS-PAGE and further confirmed by Western blotting.

         Results: PCR product (522 bp) was first subcloned into the T-Vector (replicating vector) and then successfully subcloned into the pEZZ18 (expression vector). Cloning and subclonig were confirmed by enzymatic digestion and Colony PCR. After the expression and isolation of fractions, the presence of hirudin (about 29 kDa) in different cell fractions due to the effects of signal peptide was observed in SDS-PAGE and finally confirmed by Western blotting.

       Conclusion: The gene of anticoagulant hirudin protein (desirudin) was cloned into the pEZZ18 vector containing Protein A signal peptide sequence and later transformed into E.coli JM107. The recombinant hirudin protein expression in the extracellular space was approved.

Abstract

Abstract

      Background and Objective: Agr is the most important regulatory system for the expression of Staphylococcus aureus virulence factors in different conditions. Agr acts as a quorum sensing system in this bacterium which is activated by increased cell concentration during the transition from logarithmic growth phase to stationary phase. Its role is to upregulate the secretory virulence factors such as alpha-hemolysin and inhibit the transcription of surface proteins including protein A-encoding gene. The aim of this study was to assess the relationship between the agr system expression and some virulence factors of Staphylococcus aureus in Brain-heart infusion (BHI) culture medium.

     Methods: The expression level of agrA and RNAIII genes from the agr locus along with the expression of hla, spa and mecA genes in BHI broth were assessed in different growth phases using Real time-PCR. Also, gyrB was used as an internal control in this study.

     Results: The growth curve of the five tested isolates in BHI broth at 24 hours showed that all the isolates had relatively similar growth patterns. AgrA gene expression in the stationary phase was decreased by 0.89-fold compared with the logarithmic phase. Although the expression of RNAIII gene increased by 3-fold, hla expression decreased by 0.47-fold.

     Conclusion: An inactive agr system is observed in the BHI broth medium. BHI broth medium contains high amounts of suitable nutrients for the growth of Staphylococcus aureus, thus the bacteria do not require the activity of the agr system for the regulation of the virulence genes in these conditions.

Abstract

Abstract

      Background and Objective: High-intensity interval training (HIIT) is a recently proposed exercise protocol, which is time-effective. The aim of this study was to evaluate the effect of HIIT for 8 weeks on the lipid profile, C-reactive protein (CRP), fasting blood sugar (FBS) and anthropometric parameters of young women who do not exercise.

     Methods: In this study, 20 young physically inactive women performed HIIT workouts for 8 weeks and 3 sessions per week. The training protocol consisted of 10-times treadmill running for 15 seconds at maximum effort and then 30 seconds of resting. Blood samples were taken while fasting, a day before and after the training and then the considered parameters were measured. Wilcoxon test was used to compare the obtained data.

      Results: HIIT significantly reduced FBS, cholesterol, low-density lipoprotein, high-density lipoprotein /cholesterol ratio and CRP while increasing the HDL levels. There was a significant difference in the weight, body fat percentage, waist circumference, abdominal circumference and chest circumference of the subjects before and after the training (p <0.05).

      Conclusion: HIIT can improve lipid, inflammatory and anthropometric parameters, thus it can be considered as a suitable alternative to time-consuming exercises, especially for physically inactive women who like to spend less time to achieve optimal physical wellness and body fitness.

      Background and Objective: Chlamydia trachomatis is one of the most common causes of genital infection in men and women. Genital chlamydial infections in women are clinically asymptomatic in 70-80% of the cases; therefore, the lack of timely diagnosis and treatment leads to complications such as infertility and ectopic pregnancy. The aim of this study was to evaluate the frequency of chlamydial infection in symptomatic and asymptomatic women in the Golestan province.

      Methods: This cross-sectional study was conducted on 150 cervical swab samples obtained from 150 women referred to the clinic, after obtaining written consent and completion of questionnaires. The swab samples were transferred to laboratory in phosphate-buffered saline solution and DNA extraction was carried out using phenol-chloroform and boiling methods. The frequency of chlamydial infection was evaluated by PCR.

       Results: None of the tested samples were found as Chlamydia-positive.

      Conclusion: The findings require that some more extensive research with larger sample sizes and dispersed population be performed to determine the true prevalence.  Considering the serious complications of chlamydial infections and its asymptomatic nature, a highly sensitive and specific method such as PCR should be used to detect Chlamydia. It is suggested that this method be used along with a complementary test to obtain the results that are more accurate. Furthermore, conducting simultaneous studies on other populations at risk will be very helpful in obtaining representable national data.

Abstract

ABSTRACT

       Background and Objective: Measurement of amino acids is an important tool for metabolic studies and evaluation of patients’ clinical condition. The aim of this study was to analyze the plasma amino acids using reverse phase high performance liquid chromatography techniques (RP-HPLC) with pre-column derivatization by o-phthalaldehyde (OPA) in combination with 3- mercaptopropionic acid (3-MPA).

       Methods: Overall, 107 neonates and babies suspected of having metabolic disorder were enrolled in this study. The level of amino acids in plasma samples was analyzed within 65 minutes by HPLC with pre-column derivatization by OPA/3-MPA. This was a gradient RP-HPLC method that was performed using two solvents with a ratio of methanol and sodium acetate. L-norvaline internal standard was used as the reference peak for amino acids. Standard mixture of amino acids was used to determine the concentrations of amino acids.

        Results: According to the values of coefficient of variation obtained for each amino acid, the results indicated a good chromatographic separation of amino acids by this method. The use of OPA/3-MPA derivative reagent increased the efficiency and resolution of amino acids chromatographic separation.

      Conclusion: Due to simple preparation and accurate assessment, determination of plasma amino acids using OPA/3-MPA derivatives and RP-HPLC is a suitable method in many clinical samples.
